As I've noted the last few times I've ran through Valley of Shadows, the rangers there *always* see me. How hard is it for someone with Listen/Spot to pinpoint an invisible creature?

Woops sorry. I was invisibile - AKA no stealth mode at all, just running through with the Invis spell.

How do listen/spot checks deal with sanctuary(not saved)/greater sanctuary?

I just recently added an explicit check to the OnPerceive event handler ~ if a Target has Greater Sanctuary, the monster should ignore it. I only did this for regular AI though, I may need to add it to the summon ai as well I guess.

I don't do anything special for regular Sanctuary. I could build in a seperate check with a new Will save forced everytime you reenter perception range I guess.

Are those effects behaving in any kind of consistent, describeable manner? Can it be improved?

BioWare are incompetent. There is a bug that gives monsters Infinite Range True Seeing on spawn, causing them autoattack (homing in first, if necessary) a PC regardless of Stealth, Invisibility, Greater Sanctuary, or anything else. This is also why PCs get attacked by archers from literally the entire length of an area away at times.
This bug was especially prevalent in VOS, and virtually guaranteed on almost all Sligs anywhere.

As of this morning, this bug is now fixed. biggrin.gif

(There's a similar bug to do with PCs going Invisible, HIPSing, etc, DURING combat, which I hope/expect we'll be able to tackle fairly soon now that we've identified the root of the problem, but at least that one has SOME hint of justification to it).

We may be able to leverage the GSanc workaround mentioned here http://www.dreunoctem.com/index.php?showtopic=1516 to solve those cases as well, and that's probably a lot easier than (and also probably more effective than) trying to fix half a dozen chunks of abysmal code randomly scattered throughout the AI.
